The type of Information we have: 

We hold each client’s name, address, telephone number(s), date of birth, GP, brief medical history including any allergies, current medication, the date of each foot care visit and a record of foot care procedures carried out. 

Occasionally we hold a contact number of a client’s relative or carer. 


How we get the information and why we have it:  

We ask each client to give us the above information so that we can give a professional and efficient foot care service in clients’ homes. This information may be withdrawn by the client by contacting the service. Doing this would, in effect, stop the service being delivered. 

In verbally agreeing to and giving the above information clients consent to the information being obtained and used appropriately and professionally.  


What we do with the information: 

We use the information to monitor clients’ foot health. We occasionally, with the client’s permission, contact other medical and social care providers and relatives should a concern or need arise. In an emergency we would contact appropriate emergency services. 

Information may be shared between Foot Health Practitioners working with Blackmore Vale Foot Care to enable efficiency and to obtain the professional advice/opinion of colleagues to give the best care to clients. 

Clients’ surnames may be entered, by the practitioner, onto the practitioner’s computerised accounts spreadsheet. 


How we store your information: 

The above information is written on a card(s) solely relating to each client and kept securely in a filing drawer at headquarters. It is retained for seven years after our service to the client ceases in case queries arise. After that time it is shredded. 


Your data protection rights: 

In relation to their personal information, clients have the right to: 

  • Request and be given copies of their personal information 
  • Request that information thought to be inaccurate is rectified 
  • Request that, in some circumstances, information be erased 
  • In certain circumstances, to restrict and object to the processing of the information 
  •  Request that the information held be transferred to another organisation  

To make any of these requests contact G. Walsham via the contact details above. Requests will be responded to within one month. There is no charge for exercising the above rights. 


How to complain: 

Should any client have a concern or complaint about the collection, use or retention of their personal information by Blackmore Vale Foot Care, they should contact G. Walsham using the address, telephone or email address at the top of this document.
